To apply for recertification more than 4 years after the certificate expiration date, the certificate holder shall :
1) Must begin a New Certification Process
2) Complete a Written Request to Recertify
3) Provide Evidence of 40 continuing education hours for each two year period
4) Payment of a non-refundable delinquency fee equal to three times the renewal fee ($300)
