As the FNP, you are meeting a 4-year-old patient for the first time. The child is under the care of his grandmother as his mother was incarcerated for theft recently. When examining him, you notice sparsely scattered bruises in various stages of healing on the arms and three partial thickness ~ 5mm in size on the back of the left arm. His grandmother tells you that he is clumsy and accidentally tripped into the radiator a few days ago.
A. Advise the grandmother that children this age are often clumsy and that she should put a protective cover over the radiator to prevent additional burns.
B. Call child protection services immediately and report your suspicion of child maltreatment.
C. Refer the grandmother to community agencies that can provide resources to assess the safety of the home and make recommendations to help prevent further injury.
D. Flag this as a concerning incident in the child's record and follow-up in a week to do another skin examination.
