When a client cannot read or write but is of sound mind, the nurse should read the informed consent to the client in the presence of two witnesses and the client should put X on signature line.
Informed consent is an agreement by a client to accept the treatments or procedures after being provided complete information about it (benefits and risks of treatments, alternatives and prognosis). So, if the client cannot read and write, it is important to inform him with capacity and competence to understand. The client never should feel coerced.
