The following are the actions I will take:
1. I would Firstly contact Taylor & tell that on one tube no label & on another there is only s written.
2. I would not do the test as even if I do, then the result will be varying for the two.
3. I would ask for repeat sample as test won't be done for the previous samples.
4. - Fill in the CQI (Continuous Quality Improvement) form & submit it to the Quality department for the root cause analysis.
Samples used to perform test should be very well labeled in other to avoid erroneous output whereby, the result of someone else's sample is assigned to the wrong person.
Since the tubes containing the samples are not well labeled, it would be best to contact Taylor who brought in the tubes to notify him of the inappropriate labeling.
In other to avoid doubt and ascertain the samples, it would be best to take another blood sample of the patient. Hence, carrying out the test on the initial samples won't be necessary.
Filing a report of the incident on why the test was ignored should also be documented.
