Penicillin allergy:

This is a case in which the staff mixed up the data of two patients which led to a penicillin allergy in one of the patients.
This medicine was to treat pneumonia to treat the infected patient. Incomplete data was the reason behind the problem.
The solution to this problem is to check and make sure the treatment is given to the right patient.

2. Premature discharge:

A patient is discharged before he/she is completely recovered.
A blood pressure device had a failure that was not calibrated.
This left the patient with undiagnosed hypertension.
Further, lead to readmission due to emergencies.
