The character is actually trapped in another dimension and is being surveilled (watched) by an organization? Or they are actually stuck in a coma (has a dream) and is getting experimented on. I'm not so sure, but I hope this could have given you an idea.

Maybe have them find someone they thought they would never see again?

Or if it's an apocalypse-type story have them have to fight someone they know who got infected.

Or you could go with the standard finding out there's a settlement of survivors
