The point-slope formula looks like this:

y-y1=m(x-x1)

in this case

y1=1

m=-4

x1=-3

plug in the values

Note:

For Point-Slope Form, we use a point that the line passes through and the slope of the line.

Actually, that's why it's called "Point-Slope Form" right? =)
