Complete all areas (Who you are?, Education, & Past Employment). Who you are? section should have at least 3-4 bullets. Other sections will be dependent on your personal experiences.
This section is similar to a resume so professionalism is expected.
Abbreviations are acceptable. Example: College of Coastal Georgia, ASN obtained 05/2009; Georgia Southern University RN-BSN obtained 05/2011
Present:

Complete all areas (Values, Personal Preferences, Competencies, and Limitations) with 3-4 bullets.
These areas can be a mix of personal and professional concepts.
Be honest with yourself about your competencies and limitations.
Future:

Create 3-4 SMART (specific, measurable, attainable/achievable, realistic, time-bound) goals for each section of short-term, mid-term, and long-term goals.
If you are not comfortable with intimate personal goals, they can be goals such as "Drink 64 oz of water every day".
Development needs/skills required and Action Plan sections should be 3-4 well-written sentences about your self-assessment.
