It is used to describe a wide range of approaches, tools, and techniques used to uncover the MAIN causes of problem. For example it was discovered that final year students performed woefully. On investigation, it was further discovered that the students were tutored by incompetent teachers. On further investigation, it was discovered that the school couldn't afford to hire good teachers. So it may be deduced that the root cause of the students failure is money. If a certain factor is removed from the problem system and the removal prevents the final undesirable outcome from recurring then that problem is the root cause.
